Home Support Regional Care Coordinator (North and South Wiltshire)

Join our team as a Home Support Service Regional Care Coordinator and play a key role in the day-to-day operational delivery of our Home Support Service across North and South Wiltshire.

Working as part of a team, you will be required to coordinate staffing, referrals and service delivery, supporting our teams to meet demand and ensure continuity of care.

Acting as a central point of contact for support workers, service users, carers and partners, you will help deliver responsive, person-centred support that enables people to live independently in their own homes and communities.

You will need to work flexibly across the organisation and may be required to attend or work from other sites.

Key information

Location:  Office base: Trinity House, Trowbridge, with hybrid working across offices, community and remote working.

Salary: SCP 21-23 (£26,177.94 - £27,124.04) (pro-rata per annum).

Hours: 18 hours per week
