Dementia Adviser (West/North Wiltshire)
We are looking for a compassionate, skilled Dementia Adviser to join our team in West/North Wiltshire (covering Melksham, Corsham, Box, and AWP Memory Service North).
In this rewarding role, you will provide ongoing information, advice and practical support to people diagnosed with dementia and their carers. Acting as a single, trusted point of contact, you will help individuals understand their diagnosis, navigate local services, and access the care and support they need to live well with dementia.
You will work closely with people living with dementia and their carers, using a variety of contact methods including home visits, to develop care plans, carry out regular reviews, and ensure support remains appropriate as needs change.

Must have a full driving licence and access to your own vehicle.

Salary: SCP 24 - £26,203 per annum (pro-rata)
Hours: 35 hours per-week
